The Family Support Specialist will focus on the family while addressing the needs of the individual children and adults in order to improve overall functionality, ensure family participation in the planning of services and work toward building the strengths and abilities of families. Family Support Specialists will assist children and youth in maintaining positive connections to their families; reflect, respect and respond to the cultural and linguistic strengths and needs of the child and family; and foster the integrity of the family unity whenever possible and consistent with the Department of Social Services permanent plan, without compromising the safety of its members.  

Payrate: $23.00 hourly The Case Manager is a vital member of a multi-disciplinary team that works to improve mental health functioning of youth in residential programs at YOU, Inc. The Case Manager will be responsible for providing direct care and case management services including service coordination, resource development, and supporting treatment plan implantation and aftercare follow-up, as well as working in ratio as needed depending on census. The Case Manager should attend to risk issues, documentation of care, timeliness of documentation, professionalism, caseload expectations, and administrative issues related to providing services.

To assist in the coordination of all Family Services within the assigned area in cooperation with team members, families, DDS personnel and consumers. The purpose of this position is to increase the capacity of a family to remain intact while reducing the need for residential services for people with disabilities. This position is also responsible for creating Family Support Plans that identify the areas necessary for an individual to reach their personal best. Home visits and maintaining regular and ongoing contact with the families/individuals is critical. This position will work on achieving two goals: Positive and meaningful social circle and positive and meaningful work.   Pay Rate: $22/hour
