This position is responsible for supporting the acute care admissions and clinical departments by responding to, screening, and processing all referrals and referral inquiries from statewide emergency rooms and/or mobile crisis teams in a timely manner. Once admitted, this position will follow youth through their care by providing case management services. This involves developing a relationship with the youth and their family and working closely with their Wetzel Center clinician and psychiatry providers and their external care team. This position may be required to work in the milieu as a way to understand whether referrals will be an appropriate match for care at the Wetzel Center. This position requires a strong understanding of the youth emergency mental health and acute care services in Massachusetts as well as a strong understanding of youth and family serving systems.

This includes service eligibility determination, services plan development, service coordination, resource development, as well as provide education, support, coaching, training and advocacy.
