The Behavioral Health (BH) Case Worker: Intensive Care Coordinator (ICC)-I will be responsible for providing care coordination services including service eligibility determination, services plan development, service coordination, resource development, and advocacy within the Community Service Agency (CSA). The Case Worker: ICC-I should attend to risk issues, documentation of care, timeliness of documentation, professionalism, caseload expectations, and administrative issues related to providing services that are aligned with the Principles of Wrap-Around Care.
The Behavioral Health (BH) Case Worker: Intensive Care Coordinator (ICC) II will be responsible for providing direct Intensive Care Coordination and diagnostic services that are aligned with the Principles of Wrap-Around Care to child and/or adolescent consumers and their families. The Case Worker: ICC-II will ensure that the highest-quality services are provided in compliance with performance specifications, agency policies and procedures, and regulating entities. The Case Worker: ICC-II should provide services in keeping with best practices, evidence-based knowledge, and the ethics of their educational discipline. The Case Worker: ICC-II should attend to risk issues, documentation of care, timeliness of documentation, professionalism, caseload expectations, and administrative issues related to providing wrap-around care.

$2,500 sign-on bonus
The Behavior Specialist is responsible for the development and daily oversight of behavior education services. This will include the direct training of Direct Support Staff.
$5,000 Sign-On Bonus
Under supervision of the SHNH Clinical Director, the Mental Health Clinician assesses, designs, implements, and evaluates the implementation of therapeutic interventions designed to prevent/respond to challenging behavior and to teach meaningful skills to children with developmental and/or intellectual disabilities and their caregivers. The Mental Health Clinician is a contributing member of a multidisciplinary team, collaborating with families, program staff, and other key stakeholders to improve outcomes and to promote wraparound care. The Mental Health Clinician serves as a clinical consultant to a caseload of children across settings, providing staff professional training as part of their program consultation. Maintains compliance with applicable regulations and policies and engages in ongoing professional development and systems improvement in effort to support systems-wide interventions as well as direct/indirect interventions for children.
The Mental Health Clinician is provided with a laptop, cell phone and email to perform job duties. Travel may be required.
