Provides coordination and communication in all aspects of home based therapeutic service treatment plans and P.A.S.S. plans for specified caseload in compliance with EOHHS & MCO standards. Provides general training to staff and families related to policies, procedures, forms, etc. as related to the HBTS and P.A.S.S. programs. Provides ongoing communication via telephone calls, documentation and attendance at meetings with all relevant parties involved with the specific child. Schedules and facilitates team meetings as applicable to include family members, therapists, consultants, clinical supervisor, school personnel and insurance or family service coordinator as applicable. Ensures child and family centered involvement throughout all aspects of plan development, treatment and follow-up. Position is in the Southern region of the state and would require travel to locations Providence and South.

Assists in the planning, organizing, and implementing the overall operations of the Department of Social Work in accordance with current governmental regulations and standards and with Seven Hills Pediatric Center policies and procedures, and as may be directed by the Director of Social Work to ensure that medically related emotional and social needs of the residents and their families are met.
