The Service Coordinator II is responsible for planning, scheduling, staffing, coordinating, and monitoring services for participants. Ensures that staff treats people with respect and listens carefully to them to best honor their choices, privacy, refusals and rituals while also providing appropriate safeguards. Assists people to identify and resolve any needs or concerns they may have. Assists people to learn about and advocate for their personal rights and to understand the responsibilities that accompany those rights. Ensures that all required regulations are met. Ensures that staff supports people with the kind, amount and style of assistance they need to enjoy personal well being. Recognizes that people's need for support changes as their life circumstances change and adjust supports as needed.
The Program Director for a state contracted service (see list above), will oversee the operations and management of that service area in accordance with the contract specifications. The senior director will ensure that the program operates in such a way that it meets clinical, programmatic, staff, and fiscal objectives.

The Family Support Navigator assists in the coordination of all Family Services within the assigned area in cooperation with team members, families, DDS personnel and consumers. The purpose of this position is to increase the capacity of a family to remain intact while reducing the need for residential services for people with disabilities. This position is also responsible for creating Family Support Plans that identify the areas necessary for an individual to reach their personal best. Home visits and maintaining regular and ongoing contact with the families/individuals is critical. This position will work on achieving two goals: Positive and meaningful social circle and positive and meaningful work.

The Group Home Director is provided a paid 10-day New Employee Orientation and receive training on CPR/First Aid, Crisis Prevention Intervention & Trauma-Informed Care. They also receive program-specific training at the worksite.
The Assistive Technology (AT) Specialist provides comprehensive assistive technology services, including screening, assessment, intervention, training, and outcomes measurement for persons served across affiliates of the Seven Hills Foundation. Provides and coordinates access to adapted and assistive technology to help build the independence and quality of life for people with disabilities, including people with intellectual and developmental disabilities (IDD), autism spectrum disorder (ASD), acquired brain injury (ABI), behavioral health, and/or medically intensive needs. Integrates the development, training, and implementation of the technology across selected Seven Hills Foundation points of care. Works in conjunction with Seven Hills Corporate College and the AT Program Director to develop on-site and virtual professional development opportunities for Seven Hills staff.
The AT Specialist is provided with a laptop, cell phone and email to perform job duties. Travel is required.
