The Behavioral Health (BH) Case Worker: Family Support Worker will be responsible for collaboratively engaging parents/caregivers of youth in a community setting, providing parents/caregivers with, one-to one family support, resources and referrals, and coaching that is aimed at connecting families to and building collaborative relationships with their children’s PCP, school, day care providers, counselors, Early Interventionists, and other community-based services and natural supports. The Case Worker: Family Support Worker will provide services and support in accordance with contract guidelines, including service eligibility determination, support and service plan development, , resource development, provide education, support, coaching, training and advocacy. The Case Worker: Family Support Worker should attend to risk issues, documentation of contact and support timeliness of documentation, professionalism, caseload expectations, and administrative issues related to providing services that are aligned with best practices, such as Principles of Wrap-Around Care.
