The Licensed Practical Nurse (LPN) provides direct nursing care to children and young adults with complex medical and developmental needs in a long-term care setting. This role supports resident health, safety, and quality of life while working closely with CNAs, physicians, families, and interdisciplinary staff.

Responsibilities

  • Deliver direct nursing care and administer medications/treatments safely
  • Monitor resident conditions and report changes promptly
  • Support individualized care plans and nursing interventions
  • Supervise and delegate tasks to CNAs appropriately
  • Complete accurate documentation, charting, and physician order transcription
  • Participate in admissions, transfers, discharges, and care coordination
  • Maintain medication inventory and narcotic documentation
  • Communicate effectively with families, physicians, and care teams
  • Respond appropriately to emergencies and resident concerns
  • Participate in trainings, staff meetings, and quality improvement activities
  • Strong communication, organization, and teamwork skills
  • Ability to meet physical demands of resident care in a pediatric setting

Qualifications

  • Active Massachusetts LPN license required
  • Valid driver’s license (Class C or D) required
