As a Registered Nurse in our Shared Living Program, you will play a key role in providing medical assessments, creating individualized care protocols, and offering ongoing support to program participants, staff, and caregivers. Your expertise will help ensure participants receive the highest level of health care and medical support.

 

Pay: $34-36/hour

Responsibilities

  • Develop person specific protocols and procedures for various medical conditions such as pica protocols, seizure protocols, etc.
  • Develop and train staff and caregivers on preventative and/or maintenance measures and interventions for various medical conditions/topics such as diabetes, nutrition, g-tubes, etc.
  • Assess participant’s health status and provide consultation and recommendation as needed.
  • Facilitate effective communication with participant’s physician or other emergency medical professionals to obtain medical information and treatment.
  • Complete Medication Treatment Plans for all participants requiring behavior altering medications.
  • Work in collaboration with interdisciplinary team including case manager, area director, caregiver, the Department of Developmental Services (DDS) service coordinator, etc. to ensure all health care needs are met.
  • Maintain confidential health record documentation for all participants requiring health care monitoring and consultation.
  • Become familiar and comply with all regulations set forth by the DDS.
  • Provide medical insight and recommendations during Individual Support Plan development.
  • Significant travel is required for this position.
  • Perform other duties as required or directed.

Qualifications

Education & Experience:

  • Associate's degree required.

  • Active Registered Nurse (RN) license required.
  • 3-5 years of relevant experience in healthcare, preferably with a focus on developmental or intellectual disabilities.

Skills and Knowledge:

  • Proficiency in electronic health records and computer skills a must.

     

  • Experience with developmental disability and/or intellectual disability population is a must.

  • Proficiency in electronic health records and computer systems is essential.
  • Strong communication skills and the ability to work collaboratively with interdisciplinary teams.
  • Valid driver's license (Class C or Class D).
