Pay: $20.50/hr (additional $1/hr differential for active MAP certification)

In this residential-based role, you will:
Provide person-centered supports in a congregate home setting
Treat all individuals with dignity and foster independence in daily routines
Assist with personal care, household tasks, budgeting, and skill-building activities
Implement Individual Support Plans (ISPs), Behavior Support Plans, and therapy-related supports as written
Promote meaningful community engagement and social inclusion
Support individuals in developing and maintaining relationships
Accompany individuals to medical and community appointments as needed
Collaborate with clinicians, therapists, healthcare providers, and family members
Maintain accurate documentation and data collection in compliance with agency and regulatory standards
Safeguard confidential information in accordance with HIPAA and agency policies
Communicate regularly with families/guardians and serve as a positive liaison
Participate in supervision, team meetings, and ongoing professional development
Share on-call responsibilities (typically once per month)
Perform essential personnel duties, including snow removal to maintain safe access to the residence
You are:

Education:
High School Diploma, GED, or HiSET required
Experience:
0–3 years of related experience in direct support, residential care, or human services
Licensing:
Valid Class C or Class D Driver’s License
Certifications:
CPR/First Aid certification required upon hire
Additional required trainings and certifications as assigned
