Responsibilities

  • Provide direct care and support in accordance with individualized Treatment Plans.
  • Assist with daily living activities, including personal care, nutrition, and medication administration (MAP-certified).
  • Accompany individuals to medical appointments and support their health and well-being.
  • Encourage social, communication, and community engagement skills.
  • Maintain accurate data collection and documentation to track progress and outcomes.
  • Support individuals in building and maintaining relationships with family, friends, and the community.
  • Participate in staff meetings and team communication to ensure high-quality care.
  • Assist with household and community activities, including shopping and errands.
  • Maintain certifications such as CPR, First Aid, and MAP as required.
  • Follow all agency policies and procedures, including safety, crisis response, and ethical standards.

Qualifications

  • High school diploma, GED, or HiSET required.
  • Valid driver’s license and safe driving record
