Comprehensive Healthcare Assessment: Assess, coordinate, and manage healthcare services for program participants, ensuring they receive appropriate medical attention.
Follow-Up and Oversight: Follow up on medical issues and ensure that all necessary parties (Residential Manager, Health Care Director, and other staff) are informed of any concerns.
Medical Appointments & Transport: Assist with transportation for medical appointments as required by physicians and in compliance with Department of Developmental Services (DDS) standards.
Medication Management: Oversee Medication Administration Program (MAP) compliance and ensure all reporting is up to date.
Staff Training & Support: Provide in-service training to residential staff on medical topics and contribute to residential staff meetings.
Documentation & Reporting: Maintain accurate medical records and document interventions in the TIER system as needed. Ensure compliance with all relevant regulations and requirements.
Collaboration: Communicate effectively with physicians and other healthcare providers to coordinate care and obtain necessary medical information.
Health Monitoring: Identify and report any medical conditions that may impact a participant’s progress or their placement in a residential setting.
Supervision of Nursing Staff: Supervise Licensed Practical Nurses (LPNs) if assigned to your program and provide updates to the Health Care Director regarding staff and medical concerns.
Promote Dignity & Respect: Treat all participants with respect, dignity, and cultural sensitivity, maintaining confidentiality and promoting individual growth.
General Duties: Perform other nursing responsibilities as assigned by the Health Care Director, ensuring all tasks are executed professionally and efficiently.
Education: Bachelor’s degree in Nursing required.
License: Must hold an active Registered Nursing License (RN) in Massachusetts.
Experience: 1-3 years of experience working in a residential setting with individuals with disabilities or in home care.
Certifications:
CPR/First Aid certification required.
MAP (Medication Administration Program) certification required.
Communication: Excellent verbal and written communication skills to interact with participants, staff, and medical professionals.
Driving: Valid Massachusetts driver’s license (Class C or D) required for transporting individuals as needed.
