Are you passionate about helping families build stability, resilience, and stronger connections? As a Case Manager I – Family Support, you’ll work directly with children, youth, and families to promote safety, strengthen parenting skills, and support long-term independence.

This is a hands-on, community-based role where your ability to connect, advocate, and guide families can create lasting change.

• Provide family-centered case management focused on strengthening family functioning
• Support children and youth in maintaining positive connections with their families
• Deliver culturally and linguistically responsive services to diverse populations
• Conduct home visits and provide individual, family, and group outreach
• Help families access community resources and build natural support systems
• Coach parents on effective communication, supervision, and nonviolent parenting strategies
• Advocate for families in educational, legal, and social service settings
• Assist in stabilizing children in foster care placements
• Transport children and families as needed to appointments and services
• Complete detailed documentation in electronic records for use by social workers, courts, and other professionals
• Work toward achieving service goals within established timelines
• Collaborate with multidisciplinary teams to coordinate care and services

The Ideal Candidate

You are:

• A strong communicator with excellent written and verbal skills
• Detail-oriented and able to manage documentation effectively
• Compassionate and culturally responsive
• Comfortable working independently in the community and in family homes
• Knowledgeable (or eager to learn) about community resources and parenting supports
• Bilingual (a plus, but not required)
• Flexible and able to travel extensively within the service area

• High School Diploma, GED, or HiSET required
• 0–3 years of related experience in human services, family support, or a similar field
• Valid Class C or Class D Driver’s License
• Reliable transportation with current registration, inspection, and insurance
