Seven Hills Rhode Island is seeking a highly organized and people-centered Supervisor of Conflict Free Case Management (CFCM) to lead and support a team of case managers dedicated to providing person-centered services for individuals receiving Home and Community-Based Services (HCBS). This leadership role ensures quality, compliance, and coordination of care while promoting independence and dignity for every participant.

 

Pay: $35.02/Hr

Responsibilities

  • Provide direct supervision, coaching, and oversight to case managers

  • Review case documentation to ensure accuracy and compliance with RI EOHHS and federal guidelines

  • Conduct regular one-on-one supervision, team meetings, and performance observations

  • Support implementation of person-centered plans and ensure service coordination

  • Maintain departmental training compliance and monitor adherence to timelines and regulations

  • Collaborate with state agencies and peer organizations to ensure best practices

  • Oversee departmental referrals and assignments

  • Work closely with the Area Director on planning, audits, and quality improvement

  • Uphold confidentiality and deliver high-quality customer service

  • Submit timely and accurate reports and documentation to EOHHS

Qualifications

Education & Experience:

  • Bachelor’s degree required

  • 3–5 years of relevant human services or case management experience

  • Experience with compliance monitoring and report preparation preferred

Skills & Knowledge:

  • Strong leadership, communication, and interpersonal skills

  • Knowledge of person-centered planning and service coordination

  • Valid driver’s license and ability to travel as needed
