Seven Hills Pediatric Center | An Affiliate of Seven Hills Foundation

 

Overview:
Seven Hills Pediatric Center is seeking a compassionate and detail-oriented Case Manager to join our dedicated residential team. This vital role provides case management and support services to youth in our residential programs, helping each individual achieve their highest potential in a safe, therapeutic environment. The Case Manager works collaboratively with clinical, medical, and direct care staff to ensure individualized, person-centered plans are developed, implemented, and monitored with excellence and care.

 

This position serves at the Medically Complex Behavioral Residence in Groton, Massachusetts, providing individualized, trauma-informed care for youth ages 12–18 with both complex medical and behavioral needs in a supportive and therapeutic environment.

Responsibilities

  • Provide case management, coordination, and advocacy services for youth in residential care.

  • Develop and support implementation of service and treatment plans that reflect each individual’s strengths and needs.

  • Collaborate closely with multidisciplinary teams including clinicians, nurses, and direct support professionals.

  • Coordinate care with families, caregivers, and external providers such as DCF, courts, and medical professionals.

  • Monitor and document progress, ensuring all records meet compliance and quality standards.

  • Participate in team meetings, case conferences, and aftercare planning to support safe and stable discharges.

  • Provide support in the residential milieu as needed, including working in ratio to maintain program operations.

  • Respond effectively to crises and promote trauma-informed, strengths-based care.

Qualifications

  • Bachelor’s degree in Human Services, Psychology, Social Work, or related field preferred.

  • 1–3 years of experience in residential or child behavioral health settings strongly preferred.

  • Strong organizational, communication, and problem-solving skills.

  • Ability to build rapport and work collaboratively with youth, families, and multidisciplinary teams.

  • Valid driver’s license, reliable transportation, and acceptable driving record required.

  • Ability to maintain confidentiality, professional boundaries, and ethical standards.

Certifications (to be maintained or obtained upon hire):

  • CPR and First Aid

  • Crisis Prevention/De-escalation (CPI)

  • Medication Administration Program (MAP)

  • Van Safety/Driver Training
