Direct Support Professional I - Community Health Educator

Seven Hills Behavioral Health, an Affiliate of Seven Hills Foundation

 

The Community Health Educator will be responsible to conduct client outreach and facilitate small peer-led and non-peer-led groups.  The Community Health Educator will recruit and engage populations at risk and their partners in Individual Level Interventions.  The Community Health Educator will recruit, screen and test individuals for HIV, HEP C, and STI’s.  The Community Health Educator will recruit and engage individuals and their partners on Opioid Overdose Education and Naloxone Enrollment and will assist the Program Manager or Assistant Program Manager with data collection and reporting.

 

Pay: $18.00-$20.00/Hr- Starting pay will depend on education and experience.

Responsibilities

  • Provide direct client services including:
    • Opioid overdose education & Naloxone trainings and enrollments
    • HIV/ Viral Hepatitis / STI screening and Testing
    • Syringe access and disposal
    • Referrals to health care, behavioral health care, recovery resources, social services, etc.

  • Participate in the design and preparation of client outreach recruitment activities.

  • Organize and plan weekly client outreach recruitment sessions.

  • Conduct client outreach recruitment in high risk areas.

  • Conduct Individual Level Interventions.

  • Conduct Group Level Interventions.

  • Plan and participate in community events.

  • Collect data, maintain program records and submit reports.

  • Attend project staff meetings.

  • Co-facilitate small groups on Health promotion and disease prevention for high risk populations as deemed necessary.

  • Communicate with community organizations and agencies about program activities.

  • All other duties assigned by the supervisor, his/her superior or the designee of either party.

Qualifications

Education & Experience:

  • High School diploma

     

  • Knowledge/experience in Health Promotion and Disease Prevention services and substance use and abuse issues

Skills and Knowledge:

  • Sound ability to work with all populations in an empathetic, comfortable and non-judgmental manner

  • Strong organizational skills

     

  • Ability to work as part of a team, take direction and remain flexible

  • Demonstrated skills in community education and organizing

  • Familiarity with health-related and social issues faced by high risk populations

  • Knowledge and experience providing educational outreach in public settings

  • Valid Driver’s license, ability to drive and means of transportation

  • Travel to other cities and towns is required
