Direct Support Professional I - Case Manager

As a direct support professional, you will provide essential care and support to individuals with a diverse spectrum of abilities and challenges. This may include assisting with daily living activities, promoting social and communication skills, and supporting community engagement. This role will work closely with a multidisciplinary team to implement individualized plans that align with each person's goals and needs, while maintaining quality, service excellence and ethical standards.

 

 

Pay rate: $21.80/hour

Responsibilities

Join a mission-driven team dedicated to supporting individuals with disabilities to lead fulfilling, independent lives. As a key team member, you will provide personalized, in-home and community-based support that promotes dignity, safety, and inclusion.

In this role, you will:

  • Assist individuals with daily routines, personal care, and skill development

  • Encourage independence by supporting individuals to take control of their own lives and routines

  • Promote community connections, inclusion, and meaningful relationships

  • Collaborate with clinicians, families, and support teams to implement care and behavior plans

  • Maintain accurate documentation, complete data collection, and follow individual support strategies

  • Support health and safety by attending appointments, preparing meals, and responding to seasonal needs (e.g., snow removal)

  • Participate in on-call rotation (typically once per month) and engage in ongoing training and supervision

 

Support Advocates are provided a paid 5-day New Employee Orientation and receive training on CPR/First Aid, Safety Care, and Defensive Driving. They also receive program-specific training at the work site.

Qualifications

Required Qualifications:

  • 18+ years old
  • High School Diploma or equivalent
  • Valid Driver’s License & Good Driving Record

Preferred Qualifications:

  • Commitment to assisting people with varying disabilities to participate fully in their community and in their own lives 
  • Varied interests and personal connections in the local community
  • Proven record of conscientious, responsible behavior (work, volunteer or school history)
  • Ability to work as a creative, engaged, dedicated member of your team
