Pay rate: $23/hour

Responsibilities

  • Coordinate Family Support services in collaboration with individuals, families, DDS, and community partners.

  • Create individualized Family Support Plans focused on helping individuals reach their personal best.

  • Support individuals in building meaningful social relationships and engaging in purposeful work.

  • Conduct home visits and maintain consistent, ongoing communication with families.

  • Help families stay connected and reduce the need for out-of-home residential services.

Qualifications

  • Experience in human services or a related field. (3 to 5 years experience)
  • Strong communication, planning, and advocacy skills.

  • A commitment to person-centered, strengths-based support.
