Position Overview
Are you passionate about supporting individuals to lead self-directed, fulfilling lives? As a Case Manager II at Seven Hills Rhode Island, you will play a vital role in empowering participants to make informed decisions, access essential resources, and achieve their goals through person-centered planning. This conflict-free position ensures participants receive comprehensive, unbiased support to thrive in their chosen environments.

Responsibilities

  • Facilitate and support participants in creating individualized, person-centered plans.
  • Guide participants in understanding and exercising their right to make informed choices, including self-direction.
  • Assist with Medicaid renewals and other eligibility processes to ensure uninterrupted services.
  • Provide timely, effective communication, and respond promptly to participant needs.
  • Ensure participants are aware of their rights, including the right to be free from abuse, neglect, and exploitation.
  • Monitor and reassess participant plans to align with evolving needs and preferences.
  • Collaborate with participants, their support networks, and service providers to coordinate effective, non-duplicative services.

Qualifications

  • Education: Bachelor’s degree required.
  • Experience: 3–5 years of related experience.
  • Skills: Strong communication, time management, assessment, and person-centered planning skills.
  • Knowledge: Understanding of community resources, rights and dignity, and person-centered services.
  • Other Requirements: Valid driver’s license and ability to drive.
