Are you passionate about making a difference in the lives of youth and families? We are seeking a dedicated Case Manager I - Family Support to join our team, where you'll provide crucial one-on-one support to parents and caregivers in a community setting. This role involves connecting families with resources, offering coaching, and helping build collaborative relationships with schools, service providers, and other community-based supports.

Responsibilities

  • Provide family coaching, support, and resources to help connect families with their children’s care team, including schools, counselors, and healthcare providers.
  • Develop and implement care plans, provide advocacy, and assist with service coordination and resource development.
  • Maintain a caseload, ensuring documentation and service delivery are timely and meet best practice standards.
  • Facilitate support groups, maintain the Family Resource Center (FRC) calendar, and engage volunteers for specialized groups.
  • Collaborate with community partners and attend meetings and trainings to continue personal and professional development.

Qualifications

  • High school diploma or GED required.
  • Valid driver’s license and reliable personal transportation.
  • Experience or knowledge in youth and family casework, wrap-around care, and community resources.
  • Ability to build effective relationships with clients, families, and providers.
  • Strong communication, organizational, and problem-solving skills.
  • Familiarity with child welfare, mental health, education, or juvenile justice systems is a plus.
