Overview

We are seeking a Direct Support Professional III - Senior Service Coordinator to provide direct support to individuals with developmental disabilities, working closely with program participants and their support teams. This role involves overseeing the implementation of Individual Support Plans (ISPs), managing cases, and supervising staff. The Senior Service Coordinator will ensure that all program participants receive high-quality care and assistance in achieving their goals, while also leading and mentoring staff to support the growth and development of individuals in both group and individual settings.

Responsibilities

  • Serve as a Case Manager for individuals, including maintaining case records and attending case conferences.
  • Develop, implement, and monitor Individual Support Plans (ISPs) for each participant, ensuring all requirements are met.
  • Supervise and support staff and participants in the assigned group, including training staff in therapeutic models and behavior management interventions.
  • Assist in the development of behavior management strategies in collaboration with the clinical team.
  • Foster independence and skill development in individuals by assisting with activities of daily living (e.g., personal care, meal preparation, occupational skills).
  • Provide both individual and group programming to promote age-appropriate behavior development.
  • Act as the on-site contact person in the absence of managerial staff, including opening and securing the facility.
  • Work with families, guardians, residential providers, and external agencies to track and report progress.
  • Transport participants to activities and appointments as needed.

Qualifications

  • Bachelor’s degree in Education or a related field and at least one year of experience in human services required.
  • Alternative qualifications: Associate's degree or High School diploma with two years of experience working in programs for individuals with developmental disabilities.
  • Valid driver’s license is required.
  • Experience in case management and behavior management techniques is preferred.
  • Ability to develop and implement personalized care plans and goals.
  • Strong leadership and team-building skills.
  • Excellent communication, organizational, and documentation skills.
