Pay rate: $34-$37/hour
To advocate for all medical needs of assigned program participants focusing on optimal health for all individuals in the Adult Family Care setting.
Assess and coordinate healthcare services for participants.
Assess and coordinate ancillary services such as Adult Day Health, Home Health, and other services as needed.
Review and supervise personal care services at least every 60 days.
Assess participant’s health status at least every 60 days.
Complete and update all care plans in a timely manner and with Division of Medical Assistance (DMA) regulations.
Complete semiannual reports in accordance with DMA regulations.
Maintain effective communication with participant’s physician to obtain medical information, authorization and treatment.
Report all medical and nonmedical leave of absences to the Director.
Document medical needs and interventions during each participant visit.
Advise the Director of any medical problems that may hinder a person’s progress or placement in the Adult Family Care setting.
To foster the growth of individuals and to treat them with respect, dignity and in an age and culturally appropriate manner.
Registered Nurse.
Experience with disabled/elderly adults in a community setting.
Excellent communication skills.
Valid Massachusetts drivers license.
