This role is a key part of our vision to be a leader in identifying unmet community needs, promoting individualized supports, and advocating for public policies that value and dignify all people.

As a Case Manager I - Youth Services Coordinator, you will provide essential case management services to individuals in need of support, helping them navigate behavioral health challenges with dignity and respect. You will develop and coordinate individualized service plans, advocate for resources, and ensure clients are empowered to live their lives with as much independence and well-being as possible.

Responsibilities

  • Client-Centered Service Coordination: Provide direct support to individuals with behavioral health needs by assessing eligibility, developing personalized service plans, and facilitating access to resources that promote autonomy and dignity.
  • Empowerment & Advocacy: Advocate for client needs while promoting independence, self-sufficiency, and personal well-being. Assist clients in accessing services and community activities to enhance their quality of life.
  • Ethical & Professional Care: Uphold the highest standards of ethical practice and professionalism in all interactions, maintaining respect and empathy for clients while ensuring privacy and confidentiality in line with HIPAA standards.
  • Collaborative Teamwork: Participate in team meetings, training, and supervision to enhance service delivery and maintain a collaborative, respectful work environment.
  • Documentation & Compliance: Maintain accurate and timely documentation in agency systems, ensuring compliance with agency protocols, including HIPAA and quality management standards.
  • Risk Management & Safety: Address any concerns related to client, staff, or program safety, ensuring that risks are identified and appropriately managed.

Qualifications

  • Experience: 1-3 years of experience in case management, behavioral health, or providing wrap-around care. Knowledge of mental health, educational, developmental care, child welfare, or juvenile justice systems is a plus.
  • Skills: Strong oral and written communication skills, excellent organizational abilities, and demonstrated competency in establishing therapeutic rapport and collaborating effectively with clients, families, and service providers.
  • Professional Attributes: Ability to maintain confidentiality, exercise discretion, and establish appropriate boundaries. Commitment to ethical practice and professional development.
  • Licensure: Valid driver’s license, acceptable driving record, and reliable personal transportation to fulfill essential job functions.
  • Other: Must be able to pass a background check and comply with state regulations related to driving and substance use.
