Overview

To advocate for all medical needs of assigned program participants focusing on optimal health for all individuals.

This is a direct service position providing individual medical support.

Responsibilities

•Assess and coordinate health care services for participants.  Institute appropriate follow up and communicate all medical issues to Program Manager, LPN, and other Seven Hills staff.
•Assist individuals in designated programs with their medical appointments and with transportation as recommended by the Program Manager or Nurse Practitioner.
•Serve as a resource person/mentor for LPN, C.N.A. and Child Care Workers.
•Document all nursing interventions and changes affecting the children in appropriate areas.
•Review plans of care for current knowledge of programming as required by the Department of Children and Families and/or other agencies.
•Maintain current medication and treatment orders noting any side effects and/or adverse effects and report to appropriate individuals: MD, NP, Program Manager and other designated Seven Hills staff.  Assist in training staff in medication policies and procedures.
•Maintain health records according to various government agencies as required.  Ensure that information is communicated to residential staff and school program regarding recommendations and treatment.
•Provide supervision and direction as needed to direct care staff to ensure appropriate medical health care is provided for participants.
•Attend staff meetings to present all current medical/health concerns to team as well as upcoming medical appointments.
•Complete all mandatory in-service training in a timely manner and attend any educational programs at the recommendation of the Program Manager.
•Implement procedures and services including medical interventions, or additional duties as assigned by the Program Manager or Vice President.
•Ensure that residents are sufficiently observed to maintain their safety and optimal health.

Qualifications

•Registered Nurse with current Massachusetts license.
•One year of experience working with medically fragile children.
•Valid driver’s license required.
