Provide medical oversight and health care, assessment, and promotion of overall health for people supported by Seven Hills RI. Ensures daily health care needs are met liaising with stakeholders and the community regarding ongoing health care needs. Utilizes independent judgment and operational autonomy within the field of case management.

Provides health care and promotion of good health as well as occasional triage emergencies for supported individuals.
Assists in the implementation of Federal/State laws and statues which deal with health related issues.
Develop person specific protocols and procedures for various medical conditions such as pica protocols, seizure protocols, etc.
Develop and train staff and caregivers on preventative and/or maintenance measures and interventions for various medical conditions/topics such as diabetes, nutrition, g-tubes, etc.
Facilitate effective communication with participant’s physician or other emergency medical professionals to obtain medical information and treatment.
Complete Medication Treatment Plans for all participants requiring behavior altering medications.
Work in collaboration with interdisciplinary team including case manager, area director, caregiver, DDS service coordinator, etc. to ensure all health care needs are met.
Maintain confidential health record documentation for all participants requiring health care monitoring and consultation.
Become familiar and comply with all regulations set forth by the Department of Developmental Disabilities and EEOHS.

Flexible schedule to carry out all responsibilities professionally and efficiently.

Travel throughout RI required.
To treat individuals receiving supports with respect and dignity and maintain confidentiality and integrity at all times.
To foster an environment which promotes responsibility, cooperation, consideration and hospitality.
To work in harmony and in support of the Seven Hills Foundation operating subsidiaries.
Provide supervision of nursing staff (LPN if assigned to program) and report to Health Care Director status of medical/staff concern.
Performs other duties as assigned.

LPN or RN with current RI license. Valid Rhode Island driver's license. Three years’ experience working in related support services and home care experience or case management experience.
