Overview

Position Summary:


To advocate for and institute the appropriate nursing interventions, according to the Nurse Practice Act of Massachusetts, for all medical needs of assigned program participants with a focus on optimal health for all individuals. The Northeast Rolland programs have a maximum of 5 residents in each home.


The RN Supervisor will supervise the coordination of medical services and the LPN staff in the assigned program.


This is a full-time position that includes 20 hours per week of direct care of the individuals in the assigned program. RN Supervisor will share on-call coverage (nights and weekends) on a rotating basis with the RN Supervisors of all Northeast Rolland programs.

Responsibilities

Supervisory:



  • Assess and coordinate healthcare and services for participants and institute appropriate follow-up.

  • Communicate all medical issues to appropriate members of Seven Hills interdisciplinary care team (Program staff, Resident Director, Area Director, AVP, NE Nursing Supervisor, Director of Nursing, and Clinical Coordinator).

  • Maintain, review, and supervise all medical services in assigned program.

  • Maintain health care records for all assigned individuals according to DDS standards.

  • Manage LPN staff schedule for assigned program always insuring appropriate nursing coverage.

  • Provide supervision and training to assigned nursing staff.

  • Prepare and execute annual performance reviews and monthly supervisions for all assigned nursing staff.

  • Attend, and participate in, residential staff meetings and provide in-service training to residential staff on medical topics as required or needed.

  • Review all nursing documentation to ensure all nursing interventions and changes affecting the individuals are recorded appropriately.

  • Maintain current medication and treatment orders according to MAP standards.

  • Review current ISP’s for current knowledge of support plans and strategies. Participate in annual ISP planning by updating Health Care Record and preparing annual Health Care Assessment.

  • Develop and maintain effective communication with individuals’ primary care team and health care providers.

  • Assist individuals, and transport, if necessary, on medical appointments as recommended by health care providers and DDS standards.

  • Maintain regular contact and positive relationships with individuals’ family members/guardians by providing medical status updates and notification of healthcare appointments.

  • Maintain all required license and certifications.

  • Perform additional responsibilities as assigned or directed by Northeast Nursing Supervisor or Director of Nursing.


Direct Care:



  • Provide medical support, education, and day-to-day services to individuals in assigned program.

  • Administer medications and treatments to individuals in assigned program according to Health Care Provider orders

  • Document all nursing interventions and changes affecting the individual according to the Nurse Practice Act of Massachusetts and DDS standards.

Physical Requirements:



  • Ability to drive


Ability to lift 35 pounds on occasion

Qualifications


  • Registered Nurse with current Massachusetts license.

  • Working knowledge of the medical needs of individuals with intellectual and developmental disabilities.

  • Two years of supervisory experience, preferably in a residential setting or home care environment.

  • Excellent communication skills.

  • Valid Massachusetts driver’s license.
