Overview

The Program Manager will support MCCN operations across a variety of domains including intake and referral, quality management, collateral partnerships and overall day to day performance.  Under supervision of the MCCN Director, the Program Manager will develop strategic plans and workflows to optimize service delivery and sustainability for the LTSS CP program.

Responsibilities

  • Work collaboratively and effectively with EOHHS, ACOs and MCOs to accept referrals to MCCN and ongoing management of a roster of up to 2,500 active members monthly
  • In the absence of the Director, represent MCCN in meetings with funding sources and collateral partners
  • Participate in CP workgroups to provide feedback, and develop best practices
  • Support of CP research and process improvement and staff education projects
  • Support onboarding and training of new MCCN staff members
  • Direct Supervision of select Seven Hills staff with job functions adjunct to MCCN
  • Represent MCCN Leadership in the absence of the MCCN director
  • Act as integral member of CP Operations team 
  • Other duties as assigned by MCCN director and leadership
  • Perform all duties in accordance with MCCN’s policies and procedures.
  • Strictly follow all agency Performance Standards.

Qualifications

  • B.S. in social work, human services, nursing, psychology, sociology or related field. Master’s degree preferred.
  • At least three years of experience working in the healthcare industry and with individuals with complex LTSS needs.
  • Strongly prefer candidates with experience in managed care organizations.
  • Ability to use Care Management Software to document and coordinate services.
  • Must be able to perform each essential duty satisfactorily.
  • Ability to communicate effectively verbally and in writing.
  • Strong organization skills with Attention to detail, Multi-tasking skills, Prioritization skills, Analytical skills, Problem-solving skills and Team skills.
  • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations
  • Commitment to MCCN values and mission.
  • Ability to travel.  Must have valid driver’s license and access to auto.
  • Ability to read and speak English.
