Providing social work support services for children with complex medical needs and developmental delays.  These children will primarily be under the care of the Department of Children and Families.


Assists in the planning, organizing, and implementing the overall operations of the program in accordance with current governmental regulations and standards and with Seven Hills Pediatric Center and DCF policies and procedures, and as may be directed by the Program Manager or Vice President. Provide social work services for an assigned group of residents and their families and ensure that medically related emotional and social needs of the residents and their families are met. 


Responsibilities include, but are not limited to, assuring the medical and emotional social needs of the residents and their families are met, coordinator and facilitator for interdisciplinary team on individualized care planning, maintaining appropriate social work documentation, and staff training and special projects.

Conducts home tours and provides information to family members and community professionals.

Assists in collecting diagnostic information and coordinates preadmission meetings and activities to ensure timely and smooth admissions.

Maintains regular contact with families and staff to ensure maximum family participation in residents’ lives.

Refers families to other agencies and resources as necessary or requested.

Identifies and assists financial resources for residents, their families and Seven Hills Pediatric Center at Hopedale.

Participates in service plan meeting as required.

Prepares and communicates with families/guardians individual goal summaries and planning.

Participates in medical rounds and/or departmental rounds when asked by DCF to reflect family/guardian perspectives and ensures that families/guardians are kept informed.


Master’s degree in social work from an accredited social work.

Must be licensed or license eligible in Massachusetts.


Experience working with children and young adults with developmental delays and/or complex medical needs preferred.

Experience in a healthcare setting preferred.


Computer literate.
