  • Overview

    To support individuals in appropriate employment services, through providing day to day supervision and training. To teach life management skills by way of role modeling, participation, instruction, and monitoring. To implement support per the individual’s support plan, responding to multi-disciplinary team communication.


    This is a direct service position with case management assistance, assisting individuals with disabilities through the provisions of high quality vocational and employment services. Ability to flex schedule based on the vocational support needs of program participants


    To provide job training and direct support to the individual in various employment settings including competitive, supported, integrated work sites and work services.


    To complete task analysis to be used for the evaluation and ongoing assessment of performance in the community based on employment setting.


    Maintain organized and up to date files of ISP objectives, medical fact sheets, individual learning styles, and other related documentation.


    Models healthy behavior and positive social skills


    Provides supports as written in the individual’s support plan that may include personal care, physical lifting and therapeutic holds.


    Attend and participate in meetings for the development of individual goals and objectives.


    To complete all daily documentation and data entry as assigned.


    Communicates effectively in verbal and written format. Communication is to be given and received and is every employee’s responsibility.


    To provide transportation to and from residential programs or daily work sites as assigned, using personal and agency vehicles.


    To intervene and support individuals involved in conflict or distress following procedures outlined in developed plans and Seven Hills Policy and Procedures.


    To implement vocationally geared activities and projects consistent with providing quality programming.


    To conduct relevant assessments to determine the individual interests, skills, and abilities as the individual plans their career path.


    To provide positive role modeling to individuals assigned through displaying a working knowledge of policy and procedure.


    Maintain appropriate communication with supervisors to ensure best practices.


    Performs work in compliance with Human Rights regulations.


    May work independently of direct supervision, or at times, alone.


    Maintains confidentiality and HIPAA compliance.


    Maintains and takes responsibility for certain certifications and all trainings as required, respectful of management leadership at all times.


    To ensure the individual supported receives appropriate training that will optimize their level of independence in the community.



    High school diploma or equivalent plus (1) year experience in a vocational training and/or business/industrial setting. Valid driver’s license with acceptable driving history. Able to communicate verbally, through touch, visually and in writing. Basic budgeting and mathematical skills. Proven ability to demonstrate tact, good judgment and initiative in interactions with participants, peers, family members, and the general public. Able to work with minimal supervision.
