  • Overview

    Under supervision of the Clinical Supervisor, the Therapist will deliver outpatient mental health services and supports to individuals with intellectual/developmental disabilities and/or acquired brain injuries within community-based settings (e.g., 24-hour residential programs, shared living/adult foster care placements).  The position requires proficiency with the delivery of therapeutic services and supports using a community mental health approach.  Must be able to effectively collaborate and communicate with multidisciplinary teams, which could include residential direct care, operational, medical, nursing, clinical, and/or community-based providers/agencies.  Maintains compliance with Department of Public Health (DPH) regulations.

    Responsibilities

    RESPONSIBILITIES/OBJECTIVES

    • Develop appropriate treatment plans with clients, follow them, and update them as required by regulation, payer source requirements, and client need.
    • Provide ongoing counseling to clients (individual, group, family).
    • Help clients respond constructively to treatment and assist in adjustment leading to and following discharge.
    • Provide wraparound care to members of the multidisciplinary team in order to promote continuity of care and to improve service delivery.
    • Maintain clinical documentation in compliance with state and federal laws and regulations, the requirements of various payer sources, and company policies.
    • Maintain confidential records (e.g., intakes, treatment plans, progress notes and summaries) and complete mandated reports as necessary (e.g., incident reports).
    • Perform other duties as directed.

     

    EXPECTATIONS

    • To carry out all responsibilities professionally and efficiently.
    • Adhere to the American Psychological Association’s Ethical Principles & Code of Conduct or other applicable professional code of ethics commensurate with licensure/certification.
    • To work in harmony and in support of the Seven Hills Foundation and its subsidiaries.
    • Complete and/or maintain required staff trainings, including training in a foundation-approved crisis management curriculum.

    Qualifications

    • Pass a fingerprint-based background check of the state and national criminal history databases.
    • Master’s degree, preferably in counseling, rehabilitative counseling, psychology, special education, social work, psychiatric nursing, or a related field. Professional licensure preferred (e.g., LMHC, LICSW, LCSW, CRC, special education, psychologist, psychiatric nursing).   
    • Supervised training and experience delivering therapeutic services and supports to individuals with intellectual/developmental disabilities and/or acquired brain injuries within their natural environments, including cognitive behavioral therapy, dialectical behavior therapy, anger management training, social skills training, and other evidence-based therapeutic modalities. Experience providing treatment to individuals with co-occurring mental health diagnoses or complex clinical presentations preferred.
    • Familiarity with core behavioral health documentation standards (e.g., comprehensive intake assessments, individualized treatment plans, treatment monitoring, outcome measurement, progress notes/medical necessity criteria, discharge).
    • Knowledge of human development, learning, positive behavior support, collaborative consultation, and abnormal psychology.
    • Ability to communicate effectively verbally and in writing. Ability to adjust to different environments. 
    • Valid driver’s license, ability to drive, and means of consistent transportation.
    • Travel to other cities and towns may be required.

    PHYSICAL REQUIREMENTS:  Ability to lift up to 25 lbs. on occasion.  Ability to perform primary job functions while standing for extended periods of time.  Ability to use a Foundation-approved crisis management curriculum as trained/certified.
