  • Overview

    To advocate for all medical needs of assigned program participants focusing on optimal health for all individuals.

    This is a direct service position providing individual medical support.

    Responsibilities

    • Schedules & attends medical appointments for medically fragile children.
    • Assess and coordinate health care services for participants. Institute appropriate follow up and communicate all medical issues to Program Manager, LPN, and other Seven Hills Pediatric Center at Hopedale staff.
    • Assist individuals in designated programs with their medical appointments and with transportation as recommended by the Program Manager or Nurse Practitioner.
    • Serve as a resource person/mentor for LPN, C.N.A. and Child Care Workers.
    • Document all nursing interventions and changes affecting the children in appropriate areas.
    • Review plans of care for current knowledge of programming as required by the Department of Children and Families and/or other agencies.
    • Maintain current medication and treatment orders noting any side effects and/or adverse effects and report to appropriate individuals: MD, NP, Program Manager and other designated Seven Hills Pediatric Center at Hopedale staff. Assist in training staff in medication policies and procedures.
    • Maintain health records according to various government agencies as required. Ensure that information is communicated to residential staff and school program regarding recommendations and treatment.
    • Provide supervision and direction as needed to direct care staff to ensure appropriate medical health care is provided for participants.
    • Attend staff meetings to present all current medical/health concerns to team as well as upcoming medical appointments.
    • Complete all mandatory in-service training in a timely manner and attend any educational programs at the recommendation of the Program Manager.
    • Implement procedures and services including medical interventions, or additional duties as assigned by the Program Manager or Vice President.
    • Ensure that residents are sufficiently observed to maintain their safety and optimal health.
    • Must be comfortable driving a wheelchair van.

     

    Expectations:

    • Personal care, assess respiratory status, and maintain resident’s airway while providing 1:2 supervision Monday through Friday while resident is at day habilitation.
    • Carry out all responsibilities professionally and effectively.
    • Work in harmony and in support of Seven Hills Foundation operating subsidiaries.
    • Assumes responsibility for own self evaluation and learning.
    • Formulates goals and objectives for identified areas of growth; seeks opportunities to meet own learning needs. Outlines goals annually in performance reviews.
    • Attends staff meetings and in-service education sessions.
    • Attends continuing education programs and shares information with colleagues.
    • Participates in performance improvement initiatives as directed by Quality Assurance, Coordination, Clinical Leader and DON.
    • Complies with all regulations that impact long-term care. Stay well informed of regulatory issues that directly impact nursing practice at Seven Hills at Groton.
    • Maintains good record of attendance with no more than two (2) incidences of absenteeism in ninety (90) days.
    • Reports to work unit on time and prepared to start the shift.
    • Notifies supervisor in advance in the event of absence from work.
    • Consistently completes documentation of medications and treatment administration.
    • Documents unusual events, physician and team nurse notifications, and parental notification consistently in nursing notes.
    • Communicates to supervisor and/or team nurse a complete shift report.
    • Flexibility with work schedule is expected to meet the needs of the program.

    Qualifications

    •Registered Nurse with current Massachusetts license.
    •One year of experience working with medically fragile children.
    •Valid driver’s license required.

     

    Physical Requirements

    •Must be able to lift up to 30 pounds on occasion and with assistance.
    •May be exposed to communicable diseases.
    •Annual influenza vaccination is mandatory.
    •Must pass pre-employment physical examination including drug screen.
    •Must be physically able to move sufficiently enough to respond to emergency situations.
