To advocate for all medical needs of assigned program participants focusing on optimal health for all individuals in the Adult Family Care setting.
Assess and coordinate healthcare services for participants.
Assess and coordinate ancillary services such as Adult Day Health, Home Health and other services as needed.
Review and supervise personal care services at least every 60 days.
Assess participant’s health status at least every 60 days.
Complete and update all care plans in a timely manner and with Division of Medical Assistance (DMA) regulations.
Registered Nurse. Experience with disabled/elderly adults in a community setting (i.e. internships, volunteer, or work related). Excellent communication skills. Valid Massachusetts drivers license.
