To advocate for all medical needs of assigned program clients focusing on optimal health for all individuals in a community based setting. Working with clients who are adults or seniors with developmental or medical needs in home settings.
Completing and coordinating all applicable clinical assessments and clinical evaluations.
Assess and coordinate healthcare services for participants.
Assess and coordinate ancillary services such as Adult Day Health, Home Health and other services as needed
Review and supervise personal care services at least every 60 days.
Assess participant’s health status at least every 60 days.
Developing and reviewing on an ongoing basis each member's AFC plan of care
Assisting in coordination of medical services in collaboration with Case Manager and multidisciplinary team.
Registered Nurse in state of Massachusetts.
Minimum of two years recent experience in the direct care of seniors or adults with disabliities.
